Position Responsibilities:
  • Coordinate and oversee all aspects of assigned patients' hospice care
  • Develop and implement individualized plans of care with the interdisciplinary team
  • Perform head-to-toe assessments, monitor symptoms, and manage medications
  • Educate patients and families regarding condition, needs, and care
  • Communicate patient progress with clinical leadership, physicians, and the care team
  • Complete all required documentation accurately and on time Requirements
  • Current Registered Nurse license
  • Current driver's license
  • Current CPR certification
  • Two years of Registered Nurse experience in a clinical care setting
  • One year of full-time hospice experience
