Job Description

Overall Essential Functions of the Position Comprehends and carries out the duties and responsibilities of the position in a professional and ethical manner. Builds trust-based relationships with residents, families, referral partners, and staff. Communicates clearly, compassionately, and professionally across clinical, operational, and sales functions. Utilizes sound clinical judgment in an advisory and educational capacity. Supports campus occupancy goals while honoring resident choice and dignity. Upholds resident rights, privacy, and confidentiality at all times. Do Essential Duties and Responsibilities Independent Living Clinical Support & Early Recognition (Non-Assessment) Serves as a clinical resource by responding to reported concerns or observed changes related to health, safety, function, or cognition after residency has been established Builds ongoing relationships with Independent Living residents and families to understand evolving needs and concerns over time. Provides education to support awareness of aging-related changes and future planning opportunities without conducting admission assessments or determining eligibility Transitions of Care & Internal Occupancy Pipeline Development Establishes and maintains trusted relationships with Independent Living residents and families to support future transitions to Assisted Living or Memory Care when appropriate. Serves as a consistent point of contact for families considering next steps, helping transitions feel familiar, supported, and well-timed Partners with Resident Services and Nursing Leadership to support warm handoffs after level-of-care decisions have been made. Follows residents' post-transition to reinforce trust and continuity.
Outcome:
Strong internal pipeline, improved transition experience, and higher retention within the PMMA continuum. Assisted Living & Memory Care Occupancy Support Assumes responsibility for supporting occupancy goals in Assisted Living and Memory Care, in collaboration with the Sales team. Serves as a clinical and relational resource for prospective residents and families seeking reassurance about care offerings. Builds credibility with families through education, transparency, and ongoing connection rather than transactional selling Works collaboratively with Sales to convert appropriate internal and external leads into successful move-ins. External Outreach & Relationship-Based Marketing Participates in external outreach activities such as: Hospital and referral-source meetings Community education events Professional networking opportunities Serves as a clinical ambassador for the campus, helping external partners understand care offerings and transition support. Enhances PMMA's reputation as a trusted life-plan community through presence and expertise. Sales & Occupancy Metrics Accountability Maintains responsibility for defined occupancy and pipeline metrics, aligned with measures previously associated with the Leasing Counselor role, which may include: Lead follow-up activity Internal pipeline development Move-in conversion support for AL and Memory Care Census targets as defined by leadership Documents interactions and activity in the designated CRM system. Participates in regular review of occupancy performance and strategies with leadership. Hospitalization & Family Education Support Serves as a support resource for families during hospitalization or periods of health change. Helps families understand care options and general next-step considerations without determining placement eligibility. Strengthens family confidence in remaining within the PMMA continuum. Role Boundaries The Campus Nurse Navigator does not: Conduct Independent Living admission assessments Approve or deny residency Independently determine level of care placement Provide hands-on nursing care or clinical treatment Operate with commission-based sales incentives
Requirements of Position Education & Licensure:
Licensed Practical Nurse (LPN), active and in good standing BSN preferred
Experience:
Geriatric, hospital, care coordination, or post-acute experience Experience working with families through transitions strongly preferred
