Core Responsibilities:

Conduct annual home health risk assessments in patients homes Review prior and current medical history, conditions, and medication use Educate patients on health conditions and appropriate medications Identify gaps in care and provide recommendations to enhance patient outcomes Visits may involve annual assessments or post-discharge check-ins No responsibilities for creating treatment plans or prescribing medications

Required Qualifications:

Active Nurse Practitioner license in Missouri Current CPR certification (BLS) Board certification from AANP or

ANCC MSN

from an accredited school of nursing At least 6 months of Nurse Practitioner experience is preferred
