Responsibilities Provide high-acuity nursing care to LTAC patients, including complex medical and post-ICU cases Assess, plan, implement, and evaluate individualized patient care plans Manage ventilators, tracheostomies, complex wounds, and IV medications per protocol Monitor and document patient status using EMR, reporting changes promptly to the care team Collaborate with physicians, therapists, and interdisciplinary team to optimize outcomes Educate patients and families on diagnoses, treatments, and discharge plans Adhere to infection control, safety, and LTAC regulatory standards at all times Float to units/facilities as needed, adapting quickly to varied clinical environments Required Skills LTAC nursing High-acuity patient care Ventilator management Tracheostomy care Wound care IV therapy and medication administration Telemetry monitoring Care planning and coordination Electronic medical records (EMR) Infection control
