Responsibilities:

Complete in-home annual risk assessments for Medicare Advantage plan members Review prior and current health history, medical conditions, and medications Offer patient education regarding chronic conditions and treatment regimens Identify care gaps and make informed recommendations for follow-up actions No responsibility for creating treatment plans or prescribing medications

What Youll Need:

Active Nurse Practitioner license in Missouri MSN degree from an accredited nursing program Board certification (AANP or ANCC) Current CPR certification (BLS) At least 6 months of current Nurse Practitioner experience preferred Enjoy the opportunity to travel within Perry Countyincluding Perryville, Altenburg, and Frohnawhile meeting members where they are most comfortable.
