This per diem role focuses on conducting comprehensive risk assessments for Medicare Advantage plan members, typically taking about 45 minutes per visit. Each shift may include visits to residents in Fulton, New Bloomfield, Tebbetts, Mokane, and surrounding areas, with locations varying by assignment.

Key Responsibilities:

Qualifications:
