Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage health plan members Review and evaluate patients previous and current medical histories, ongoing conditions, and medication regimens Provide clear education and counseling regarding diagnoses and medications Detect care gaps and recommend follow-up or interventions as needed Focus solely on assessmenttheres no requirement to create treatment plans or prescribe medications

Qualifications:
