You will conduct in-home visits, performing comprehensive risk assessments that take approximately 45 minutes each and may include annual health evaluations or post-discharge visits. Your scheduled assessments may be based in Boonville and nearby areas such as Wooldridge, Blackwater, and Otterville.

Key Responsibilities:

Perform detailed annual risk assessments for Medicare Advantage health plan members Review and evaluate patients medical histories, current conditions, and medication regimens Educate patients on their diagnoses and prescribed treatments Identify gaps in care, providing actionable recommendations to improve patient outcomes Role does not involve developing treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner License in Missouri Board Certification through AANP or

ANCC MSN

degree from an accredited nursing program Current CPR (BLS) certification At least six months of recent nurse practitioner experience preferred In addition to a highly competitive evaluation rate of $80$90 per completed visit, youll also receive a mileage stipend and compensation for administrative tasks performed.
