Essential Responsibilities:

Conduct comprehensive annual home health risk assessments for Medicare Advantage health plan members. Review and evaluate patients prior and current medical and medication histories. Provide clear education to patients regarding their medical conditions and prescribed medications. Identify gaps in care and offer recommendations to enhance patient outcomes. No requirement for developing treatment plans or prescribing medications.

Desired Qualifications:

Current Nurse Practitioner license valid in MO. Active CPR certification (BLS). National board certification (AANP or ANCC). MSN degree from an accredited nursing program.
