This per diem opportunity centers on evaluating Medicare Advantage plan members throughout Perry County, MO, including Perryville, Altenburg, and Frohna. Each home health risk assessment visit, on average, takes about 45 minutes and may involve an annual health assessment or a post-discharge evaluation based on the members specific needs.

Key Responsibilities:

Perform comprehensive annual home health risk assessments for Medicare Advantage plan members Review prior and current medical histories, chronic and acute conditions, and current medication use Educate patients about medical conditions, preventive measures, and medication regimens Identify care gaps and provide actionable recommendations to close them No requirement for developing treatment plans or prescribing medications

Qualifications:

Current and unrestricted Nurse Practitioner license in MO Active CPR certification (BLS) Board certification through either AANP or ANCC Minimum of 6 months of recent practical NP experience preferred MSN from an accredited nursing program This per diem role offers competitive pay of $80-$90 per evaluation, along with a mileage stipend and compensation for administrative time.
