Key Responsibilities:

Perform comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members Review and document past and current medical histories, conditions, and medication use Educate members and families about ongoing conditions and prescribed medications Recognize care gaps and recommend next steps; no need for prescribing medications or creating treatment plans

Qualifications:

Active Missouri Nurse Practitioner license Board Certification from AANP or

ANCC MSN
