Key Responsibilities:

Conduct thorough, 45-minute home health risk assessments for Medicare Advantage plan members Review medical histories, current and prior conditions, and medication usage Provide patient education on health conditions and medications Identify care gaps and make recommendations to close those gaps Focus solely on assessmentno prescribing or treatment planning required

Qualifications:

Current Nurse Practitioner (NP) license in Missouri MSN degree from an accredited school of nursing Board Certification through AANP or ANCC Current CPR certification (BLS) At least 6 months of recent NP experience preferred This position offers strong earning potential at $80-$90 per evaluation, plus a stipend for mileage and time spent on administrative duties.
