A valuable opportunity is available for a dedicated Nurse Practitioner with home health experience to perform comprehensive risk assessments for Medicare Advantage plan members. Working per diem, you will conduct in-home evaluations throughout Cass County, MOincluding Harrisonville, Raymore, and Clevelandas well as Kansas City and surrounding areas. This flexible position allows you to set your own schedule, accommodating shifts between Monday and Sunday with minimum weekly or bi-weekly hourly commitments.

Key Responsibilities:

Conduct annual home health risk assessments for Medicare Advantage health plan members Review and evaluate prior/current medical history, conditions, and medication usage Educate members about their health conditions and prescribed medications Identify gaps in care and recommend actions for improved health outcomes Visits may include annual assessments or post-discharge evaluations No diagnosis, treatment planning, or prescribing required

Qualifications:

Active Nurse Practitioner license in Missouri Current CPR certification (BLS) Board certification through AANP or ANCC Minimum 6 months of experience as a Nurse Practitioner preferred Master of Science in Nursing (MSN) from an accredited institution You will be compensated at $80-$90 per evaluation, with additional mileage stipend and payment for administrative time.
