Key Responsibilities:

Conduct annual and post-discharge home health risk assessments (approximately 45 minutes each) in patient residences Review and document medical history, current medical conditions, and medication usage Educate patients on conditions and medication management during visits Identify gaps in care and offer recommendations for care improvement No responsibility for developing treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner License in Missouri Master of Science in Nursing (MSN) from an accredited institution Board Certification through AANP or ANCC required Current CPR certification (BLS) Minimum of 6 months Nurse Practitioner experience preferred Enjoy highly competitive earnings of $80$90 per evaluation, plus a mileage stipend to support travel throughout Benton County, and compensation for administrative time.
