Key Responsibilities:

Perform comprehensive annual risk assessments in patients' homes for a Medicare Advantage health plan Review and evaluate prior and current medical history, conditions, and medication usage Educate patients about their medical conditions and prescribed treatments Identify care gaps and recommend strategies for optimal patient outcomes Focus on assessmentno treatment planning or medication prescribing required

Qualifications:

Active and unrestricted Nurse Practitioner license in Missouri Current CPR certification (BLS) Board certified as a Nurse Practitioner through AANP or ANCC At least six months of NP experience preferred Master of Science in Nursing (MSN) from an accredited institution This rewarding per diem position provides compensation of $80-$90 per completed evaluation, including a mileage stipend and paid administrative time.
