Schedule & Hours:

Monday Friday, 8:00 AM 5:00

PM Caseload:

30 points per week (typically 35 patient visits per day)

Key Responsibilities:

Conduct comprehensive OASIS assessments and develop individualized care plans. Deliver skilled nursing services, such as wound care, medication administration, IV therapy, and chronic disease management. Monitor patient status, identify changes in condition, and promptly collaborate with the care team and physicians. Educate patients and their families about disease processes, treatment, medication, and preventative care. Document all patient interactions and care activities using Home Care Home Base (HCHB). Work closely with physical therapists, occupational therapists, social workers, and home health aides for coordinated care. Participate in case conferences, staff meetings, and ongoing training sessions. Ensure compliance with all regulatory standards and best practices for patient care.

Qualifications & Experience:

Active RN license in good standing for the state of practice. At least one year of recent Home Health field experience. Proficiency in Home Care Home Base (HCHB) and OASIS assessment required. Reliable transportation, valid drivers license, and auto insurance required.
