Schedule:

13-week contract with potential for extension Full-time, Monday through Friday On-call likely required in addition to regular weekday hours Structured patient visit expectations each day

Key Responsibilities:

Deliver in-home nursing services and utilize skilled clinical interventions Complete detailed patient assessments with precise OASIS documentation, adhering to deadlines Partner with physicians and support teams to establish and carry out personalized care plans Perform wound management, medication administration, patient teaching, and disease monitoring Report patient conditions and care updates to the multidisciplinary clinical team

Qualifications & Desired Experience:

Current, unrestricted RN license At least 1 year of recent experience as a home health RN Demonstrated expertise in OASIS documentation Familiarity with HomeCare HomeBase (highly preferred) Strong organizational skills and reliability with structured scheduling Valid drivers license and dependable transportation
