Deliver high-quality in-home skilled nursing services Complete patient assessments and OASIS documentation accurately and punctually Collaborate with healthcare providers to implement individualized care plans Administer wound care, medication management, patient teaching, and disease monitoring Provide consistent updates regarding patient status to the clinical team

Qualifications:

Active RN license At least one year of recent home health RN experience Proven OASIS documentation proficiency Familiarity with HomeCare HomeBase (preferred) Ability to manage a structured, full-time schedule and daily patient visits Reliable transportation and a valid drivers license
