As a Home Health RN in this region, you will support patients in their homes across Forest and the surrounding rural and semi-rural communities. You will be part of a collaborative clinical team focused on helping individuals remain safely at home, manage chronic conditions, and recover from acute illnesses or surgeries. Typical responsibilities include performing initial and ongoing skilled nursing assessments, developing and updating individualized plans of care, administering medications and treatments, providing wound care, monitoring vital signs and symptoms, and offering education to patients and family caregivers on disease processes, self-care techniques, and safety in the home. You will coordinate closely with physicians and other team members to communicate changes in condition, adjust care plans, and ensure seamless continuity of care. A typical day in this role involves traveling between patient homes, conducting scheduled visits, documenting findings and interventions in the EMR, and participating in team case conferences. You will manage a caseload that allows you to spend meaningful time with each patient, building long-term therapeutic relationships and seeing the impact of your care over time. Patient acuity and visit frequency vary based on clinical needs, with a mix of post-acute, chronic disease, and rehabilitative cases common in the home health setting. While there are no traditional facility bed counts in home health, you will work within an organized structure that supports safe staffing and manageable visit volumes. Shifts generally follow daytime hours with some flexibility based on patient scheduling and agency needs, and may include occasional weekend or on-call rotation to provide continuity of care. The role requires comfort working independently in the field, sound clinical judgment, and a strong commitment to patient-centered care.
