Key Responsibilities:

Conduct thorough annual home health risk assessments, typically lasting about 45 minutes, in patients' homes Review prior and current medical history, chronic conditions, and medication regimen Educate patients on medical conditions, medications, and appropriate self-care strategies Identify and address gaps in care, offering recommendations for improving health outcomes No treatment plan development or prescribing requiredfocus on assessment, education, and care navigation

Qualifications:

Active Nurse Practitioner License in Mississippi Board Certification through AANP or ANCC Master of Science in Nursing (MSN) degree from an accredited institution Current, valid CPR (BLS) certification At least 6 months of current nurse practitioner experience preferred
