Seeking a dedicated healthcare professional to provide home health risk assessments for Medicare Advantage plan members in Washington County, MS. This per diem opportunity offers flexibility, allowing you to set your own schedule and shifts between Monday and Sunday. Assignments may take place in various local areas, including Greenville, Leland, and Hollandale. You'll play a key role in delivering comprehensive evaluations, supporting patients health management while enjoying the autonomy of self-directed scheduling.

Responsibilities:

Perform annual and post-discharge home health risk assessments for Medicare Advantage plan participants Review patients previous and current medical history, existing conditions, and medications Educate patients regarding their health conditions and prescribed medications Identify and document any care gaps, supplying recommendations to address them No responsibility for creating treatment plans or prescribing medications

Qualifications:

Nurse Practitioner license in Mississippi Current CPR certification (BLS) Board certified via AANP or ANCC At least 6 months of recent Nurse Practitioner experience (preferred) Masters degree in Nursing (MSN) from an accredited institution
