Key Responsibilities:

Perform annual health risk assessments for Medicare Advantage plan members in home settings Review and evaluate prior/current medical histories, conditions, and medication regimens Educate members about their health conditions and prescribed medications Identify gaps in care and suggest actionable recommendations Complete thorough assessmentsno treatment planning or prescribing required Average assessment duration: approximately 45 minutes

Qualifications:

Active Nurse Practitioner license in MS Current CPR certification (BLS) Board Certified through AANP or ANCC Minimum of 6 months recent experience as an NP preferred Master of Science in Nursing (MSN) from an accredited program

Compensation and Benefits:
