PM Key Qualifications and Requirements:

Current, active RN license in good standing for the state of practice At least one year of recent home health nursing experience (field experience strongly preferred) Proficiency with Home Care Home Base (HCHB) electronic documentation system Experience conducting and documenting OASIS assessments Reliable vehicle, valid drivers license, and up-to-date auto insurance

Role Responsibilities:

Conduct thorough OASIS assessments and develop individualized patient care plans Deliver skilled nursing care in the field, including wound care, medication administration, IV therapy, and chronic disease management Monitor and communicate changes in patient condition to ensure appropriate care adjustments Educate patients and families on disease management, medications, and preventive care strategies Accurately document all patient encounters in HCHB system Collaborate with a multidisciplinary teamphysical therapists, occupational therapists, social workers, and aidesto coordinate comprehensive care Attend case conferences, staff meetings, and continuing education sessions as required Ensure compliance with all agency and regulatory standards for patient care and documentation Manage a caseload averaging 35 patient visits daily, with a weekly target of 30 points Travel daily within assigned territory (mileage reimbursement provided)
