Primary Responsibilities:

Conduct home health risk assessments, including annual and post-discharge visits, for Medicare Advantage plan participants in their residences Review and evaluate patients medical history, current health status, and medication usage Educate patients on their medical conditions and proper medication management Identify gaps in care and recommend next steps to ensure optimal patient health No prescribing, treatment plan creation, or on-call duties

Qualifications:

Active Nurse Practitioner license in Mississippi Current CPR certification (BLS) Board Certification through AANP or

ANCC MSN

degree from an accredited nursing program At least 6 months of recent Nurse Practitioner experience preferred This role offers competitive per-evaluation compensation ranging from $80-$90, plus a mileage stipend and reimbursement for administrative time associated with your visits.
