Qualifications:

Active and unencumbered RN license Minimum of 1 year recent experience as a home health RN Proven OASIS assessment and documentation skills Familiarity with HomeCare HomeBase (strongly preferred) Reliable transportation and current drivers license Comfortable handling daily patient visits and maintaining an organized caseload

Role Responsibilities:

Deliver skilled in-home nursing care and clinical interventions Complete timely and accurate OASIS assessments and care documentation Collaborate efficiently with physicians, therapists, and the broader care team to establish and revise care plans Provide wound care, medication management, disease monitoring, and patient/family education Communicate significant changes in patient condition promptly to the appropriate clinical contacts
