Key Responsibilities:

Complete annual home health risk assessments in patient residences, averaging 45 minutes each Review and evaluate patients prior and current medical history, ongoing medical conditions, and medication records Educate patients about their diagnoses, treatment, and prescribed medications Identify care gaps and provide actionable recommendations to enhance patient outcomes Focus on assessmentcreation of treatment plans and prescribing are not required

Qualifications:
