Key Responsibilities:

Complete comprehensive annual home health risk assessments for Medicare Advantage members Review and evaluate prior and current medical history, existing medical conditions, and medication usage Educate patients regarding their health status and prescribed medications Detect care gaps and recommend steps to improve patient outcomes No responsibility for developing treatment plans or prescribing medications

Qualifications:
