We are seeking a dedicated Registered Nurse (RN) with strong home health experience and proficiency in OASIS documentation for a 13-week full-time contract. Schedule:

Full-time, Monday through Friday On-call participation may be required Daily patient visit structure ensures consistency and clear expectations

Key Responsibilities:

Deliver comprehensive in-home nursing care, including wound care, disease monitoring, and medication management Complete detailed patient assessments and OASIS documentation promptly and accurately Collaborate with physicians and interdisciplinary care teams to develop, implement, and adjust individualized care plans Communicate changes in patient condition and progression to the clinical team in a timely manner Educate patients and families on disease management and self-care strategies

Qualifications:

Current and active RN license Minimum of 1 year of recent home health RN experience Demonstrated expertise with OASIS documentation Experience with HomeCare HomeBase (strongly preferred) Valid drivers license and reliable transportation Ability to effectively manage full-time, structured patient visit schedules
