Schedule:

Monday Friday, 8:00 AM 5:00

PM Caseload:

Expectation of 30 points per week (typically 35 patient visits per day) Daily travel within an assigned territory (mileage reimbursement provided)

Responsibilities:

Conduct comprehensive OASIS assessments and develop individualized care plans Deliver skilled nursing care: wound care, medication administration, IV therapy, and chronic disease management Evaluate patient progress, report changes in condition promptly, and communicate with interdisciplinary care teams and physicians Provide patient and family education on disease processes, medications, and preventive strategies Accurately document all care in Home Care Home Base (HCHB) Work collaboratively with therapists, social workers, and home health aides for cohesive patient care Attend case conferences, staff meetings, and relevant continuing education sessions Uphold all regulatory and agency standards regarding documentation and patient care

Qualifications:

Active RN license in the state of practice, in good standing Minimum of one year recent field-based Home Health experience Proficient in Home Care Home Base (HCHB) and OASIS assessments Reliable transportation, current drivers license, and valid auto insurance
