We are seeking a dedicated Registered Nurse (RN) with proven home health experience and OASIS documentation skills for a full-time, 13-week contract assignment. Schedule:

13-week contract with the possibility of extension Full-time, Monday through Friday On-call availability may be required

Key Responsibilities:

Deliver skilled, compassionate in-home nursing care, including patient assessments, wound care, medication management, disease monitoring, and education Accurately complete OASIS documentation and submit timely reports Collaborate effectively with physicians and interdisciplinary care teams to develop and manage individualized care plans Maintain regular communication about changes in patient condition Adhere to assigned daily visit expectations and prioritize quality care

Qualifications:

Active RN license At least one year of recent RN home health experience OASIS documentation proficiency required Strongly prefer experience with HomeCare HomeBase EMR Strict adherence to a structured, full-time schedule requiring daily patient visits Reliable vehicle and current drivers license
