$2,646(Including stipend for travellers) Job Description We are seeking an experienced Hospice RN / Case Manager to provide skilled nursing care and case management to patients in their homes or residential settings. The RN will coordinate patient care, support patients and families, and collaborate with an interdisciplinary healthcare team to ensure continuity and quality of care. This position may initially begin as a Visiting RN and transition into a Case Manager role. Responsibilities Provide skilled nursing care to hospice patients in their place of residence. Assess patients' physical and psychosocial conditions and develop appropriate care plans. Monitor changes in patient conditions and responses to treatments and medications. Coordinate care with physicians, interdisciplinary teams, community agencies, and family members. Provide patient and family education, counseling, and symptom management. Supervise and coordinate care provided by Home Health Aides and CNAs. Maintain accurate and complete patient documentation. Participate in case conferences, care coordination, and quality improvement activities. Participate in the on-call and weekend rotation as required. Requirements Active Montana RN license required. Valid driver's license, reliable transportation, and appropriate auto insurance required. 2 years of recent acute care, home health, or hospice experience preferred. Experience with venipuncture, catheterization, wound care, and parenteral therapy preferred. HCHB (Homecare Homebase) experience is strongly preferred. BSN preferred.

Pay:

$2,400.00 - $2,646.00 per week
