8:00•4:30 pm Monday•Friday (excluding federal holidays) Additional Details about the position 4-8 ppd (depending on case load) Providing specialized case management and care coordination for patients with special health care needs. Reviewing and monitoring outpatient consultations and referrals for medical necessity, cost effectiveness, and appropriateness. Collaborating with PRC staff, providers, and external facilities to ensure care aligns with CMS criteria. Maintaining statistical data on consults, monitoring deferred referrals, and supporting PRC team continuous quality improvement.

CERTIFICATION REQUIREMENTS BLS EXPERIENCE REQUIREMENTS

case management experience IHS experience is a plus MUST have Purchased Referred Care (PRC) experience

LICENSURE REQUIREMENT

Any State RN License
