A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Chenega Government Mission Solutions, LLC, a Chenega Professional Services' company, is looking for a Registered Nurse (RN) - Case Manager (Purchased/Referred Care) in support of Indian Health Services within the Fort Peck Service Unit in Wolf Point and Poplar, MT. Responsibilities Providing specialized case management and care coordination for patients with special health care needs. Reviewing and monitoring outpatient consultations and referrals for medical necessity, cost effectiveness, and appropriateness. Collaborating with PRC staff, providers, and external facilities to ensure care aligns with CMS criteria. Maintaining statistical data on consults, monitoring deferred referrals, and supporting PRC team continuous quality improvement.
Schedule:
Monday-Friday, 8am-4:30pm (excluding federal holidays). Qualifications At least 3 years of experience as an Registered Nurse, with case management and Purchased/Referred Care (or similar) experience. Permanent, current, full and unrestricted license to practice a Registered Nurse in a state, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Ability to obtain an IHS public trust clearance. Estimated Salary/Wage USD $34.01/Hr. Up to USD $43.60/Hr.
