Job Description
Case Manager Registered Nurse Join our team as a Case Manager Registered Nurse and make a meaningful impact on patient care through comprehensive case management. This role offers an excellent opportunity to utilize your nursing expertise in a dynamic environment, coordinating care across inpatient, outpatient, and community settings to promote optimal wellness and continuity of care.
Overview:
As a Case Manager RN, you will participate in all phases of the Case Management Program, ensuring adherence to established standards of care. Your responsibilities include conducting assessments, developing and implementing treatment plans, coordinating services, and monitoring patient progress. You will collaborate with multidisciplinary teams and community agencies to facilitate seamless transitions and comprehensive care delivery, all within a supportive and structured schedule.Required Skills:
Minimum Associate Degree in Nursing from an accredited institution Current, unrestricted RN license from any state Certification in one of the following: CCM, CDMS, CRRN, COHN/COHN-S, ACCC, CRC, RN-NCM, or CMC At least one year of nursing experience post-graduation Ability to pass a government background check/security clearance Current BLS Healthcare Provider certification from AHA or ARC Strong assessment, planning, coordination, and documentation skills Excellent communication and interpersonal skills Nice toHave Skills:
Experience with case management software and databases Knowledge of community resources and social services Familiarity with medical care criteria such as InterQual Experience working with military or veteran populations Ability to participate in telehealth and virtual meetingsPreferred Education and Experience:
Bachelor's Degree in Nursing or higher is advantageous but not mandatory More than one year of nursing experience, especially in case management or related fields Prior experience working within multidisciplinary teams or community health settingsOther Requirements:
Work schedule: Monday to Friday, 8.5 hours per day, between 7:00 am and 5:30 pm No weekends, holidays, on-call, or call-back duties Ability to travel or transfer patients regionally as needed Willingness to participate in teleconferences and team meetings This is a rewarding opportunity to advance your nursing career while contributing to vital patient care initiatives.
