General Statement of Duties The Public Health Nurse functions as a key leader within the Polk County Consolidated Health and Human Services Agency (HHSA) Public Health Division. This position provides programmatic leadership for school health, communicable disease prevention, and public health nursing initiatives across Polk County. This role oversees the coordination, implementation, and evaluation of public health nursing programs through collaboration with local education agencies, healthcare providers, and community partners. The Program Manager supports an integrated care model that leverages HHSA services, community partnerships, and evidence-based public health practice to improve population health outcomes. Primary responsibilities include program planning, oversight of communicable disease response activities, school and child health coordination, staff development, and quality improvement initiatives. While grounded in professional nursing practice, the position focuses primarily on program management, system coordination, and strategic partnership development to ensure effective delivery of public health services. Distinguishing Features of the Position Work is performed in accordance with the North Carolina Nurse Practice Act, North Carolina Pharmacy Act, North Carolina Public Health Nurse dispensing rules, agency regulations, program policies, provider orders, and evidence-based public health nursing practice. The Public Health Nurse performs a variety of clinical, coordination, and program support duties requiring knowledge of nursing theory, public health principles, and communicable disease control practices. The position supports the management of communicable disease activities and public health programs and assists with program planning, coordination, and quality improvement for accreditation. Public Health is responsible for preparedness activities during natural disasters and disease outbreaks. This position participates in preparedness planning and response activities, which may include supporting mass vaccination clinics, shelter operations, medication dispensing from the Strategic National Stockpile, and other emergency response functions. The employee must demonstrate the ability to integrate clinical knowledge with program management responsibilities, interpret and align work with public health data and policy, and collaborate with community partners and internal teams to address health needs in Polk County. Minimum Requirements and Experience Associate Degree in Nursing required. Bachelor's or Master's Degree in Nursing preferred. Experience in public health, school health, or communicable disease programs preferred. Current license to practice as a Registered Nurse in North Carolina issued by the NC Board of Nursing. Successful pre-employment drug screen. Successful criminal background check. •All HHSA employees may be called upon to perform work during emergency or disaster situations, including staffing shelters overnight. These duties will take precedence over routine responsibilities when activated. Physical Requirements The ability to perform all duties associated with the position including, but not limited to lifting, prolonged standing, driving, sitting, bending, stooping, and operating or performing work with associated tools and machines.
