A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Job Description:
Schedule:
Part Time•8a-4:30p with rotating Saturdays Creates plan for care across the continuum, integrating patient/family preferences and values. Monitors patient care through assessments, evaluations and/or patient records. Advocates for resources and removal of barriers. Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan. Acts as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care."
Education:
Associate's of Nursing [Required] Bachelor's of Nursing [Preferred]
Licenses and Certifications:
Registered Nurse (RN) [Required] Basic Life Support•CPR Cert (BLS) [Preferred]
