Key Responsibilities:

Perform comprehensive annual and post-discharge risk assessments for Medicare Advantage plan members in their homes Review and document patients prior and current medical history, chronic conditions, and medication use Educate patients on health conditions, wellness strategies, and their ongoing medication regimens Identify gaps in care and provide actionable recommendations for follow-up and care coordination No requirement for developing treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in North Carolina Board Certification as NP (AANP or ANCC) Current CPR certification (BLS) MSN from an accredited nursing program At least 6 months experience as an NP preferred Home health experience with a commitment to compassionate care Enjoy premium compensation with $80-$90 per completed evaluation, plus stipends for mileage and administrative time.
