Well Care Home Health is seeking a Home Health Registered Nurse to deliver compassionate, one-on-one care to patients in their homes across North Carolina. In this role, you will perform comprehensive assessments, provide skilled nursing services such as wound care, IV therapy, and medication management, and develop individualized care plans in collaboration with physicians and an interdisciplinary team. You will educate patients and families on disease management and safety, document care in the EMR, and coordinate services to support recovery, chronic condition management, and independence.
