Responsibilities:

Perform annual home health risk assessments for Medicare Advantage members Assess patients prior and current medical history, existing conditions, and medication usage Teach patients about their health conditions and prescribed medications Identify gaps in care and make recommendations to improve patient outcomes No treatment plan creation or medication prescribing required

Qualifications:
