A Nurse Case Manager applies nursing expertise to coordinate a patient's medical care and provides advocacy to help them get the best and most affordable care. Develop care plans including discharge planning, coordinate delivery among providers, and teach patients and families how to follow the plan. They may also help patients navigate insurance and other healthcare bureaucracies and access related social services. May work with patients who have chronic illnesses or other complex medical needs. Works at a hospital or other healthcare facility; may follow patients throughout the span of their treatment.

Job Description

The Shannon Gray Rehabilitation and Recovery Center is seeking an experienced MDS nurse. MDS/RAI experience is required. Our facility has 150 skilled beds and facilitates a great deal of short term rehabilitation back into the community. Extensive RUG management experience and knowledge, Long Term Care Case Management, Case Mix knowledge, and excellent assessment skills are a must. Excellent communication and interpersonal skills for effective patient/family/team interaction. Compassionate approach towards patient care with a focus on empathy and support. TEAM oriented with a desire to uplift and educate others. Dependability is a MUST.
