Key Responsibilities:

Perform comprehensive annual home health risk assessments for eligible Medicare Advantage health plan members Collect and evaluate prior and current medical histories, clinical conditions, and medication use Offer education and information to patients about their conditions and prescriptions Identify potential gaps in care, providing clear recommendations for follow-up or resolution Visits may include annual health reviews or post-discharge assessments No ongoing treatment plan development or medication prescription is necessary

Qualifications:

Active Nurse Practitioner license in North Carolina Board Certification through AANP or

ANCC MSN

degree from an accredited nursing program Current CPR (BLS) certification required 6 or more months of recent NP experience is preferred
