Key Responsibilities:

Perform comprehensive annual home health risk assessments for Medicare Advantage members in their homes Assess and document prior and current medical history, relevant conditions, and medication usage Educate patients on conditions and best practices for medication and self-care Identify gaps in care and offer recommendations to address those needs Visits last approximately 45 minutes and may include annual assessments or post-discharge visits No need to develop treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in North Carolina Board Certification through AANP or ANCC Current CPR certification (BLS) MSN degree from an accredited school of nursing Minimum six months of recent NP experience preferred As part of this per diem opportunity, you will receive compensation of $80$90 per evaluation along with a mileage stipend and administrative pay to acknowledge your time.
