Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage members Review previous and current medical conditions, medical history, and medications Educate patients about their health conditions and the safe use of medications Identify potential care gaps and recommend actions to improve health outcomes No responsibilities related to treatment planning or medication prescribing

Qualifications:
