Key Responsibilities:

Perform detailed annual home health risk assessments Review and document patients' prior and current medical history, conditions, and medication usage Provide education to patients about their medical conditions and prescribed medications Identify care gaps and recommend solutions to optimize patient outcomes Please note: No treatment plan development or medication prescribing is involved

Qualifications:

Current and valid Nurse Practitioner license in North Carolina MSN degree from an accredited nursing school Board Certification via AANP or ANCC Active CPR certification (BLS) At least six months of recent Nurse Practitioner experience preferred
