Job Overview We are seeking an energetic and detail-oriented Medicare Wellness Nurse - Licensed Practical Nurse to join our team. In this vital role, you will focus on providing preventive health assessments and wellness visits for Medicare-eligible patients. Your expertise will help improve patient outcomes by conducting thorough evaluations, educating patients on health management, and coordinating necessary follow-up care. This position offers an exciting opportunity to make a meaningful impact on patient lives while working within a supportive healthcare environment. Responsibilities Conduct Medicare wellness visits, including comprehensive health assessments and screenings Educate patients on preventive care, chronic disease management, and healthy lifestyle choices Review and update patient medical records with accurate documentation of assessments and recommendations Coordinate follow-up care and referrals with primary care providers and specialists as needed Perform vital signs measurement, medication reconciliation, and review of current health status Maintain compliance with HIPAA regulations and ensure confidentiality of patient information
