Job Description

Job Summary:
The Medicare Wellness nurse works in collaboration with the primary care provider and all members of the health care team to ensure the highest level of service is provided. Primary responsibilities include identification of Medicare patients appropriate for Annual Wellness Visits (AWV) and responsible for conducting the initial and annual preventive care visits. Assesses healthcare needs that require additional follow-up by primary care provider and assists patients in appropriate follow-up. Knowledgeable of community resources and assists patients in connecting with those resources. Responsible for implementing the philosophy, objectives, and standards of care of the Medicare annual wellness program. All duties and responsibilities must be carried out with empathy and compassion towards patients and their families, as well as coworkers. Minimum Requirements:
  • Graduate from an approved school of Nursing.
  • Current RN license with North Carolina State Board of Nursing or Compact License recognized by the North Carolina State Board of Nursing.
  • One year of nursing experience required, 2 years preferred.
  • Must maintain current AHA BLS certification.
  • Demonstrate knowledge of current trends and techniques in nursing.
