Competitive, based on experience About the Opportunity A well-established Primary Care practice serving the Pineville and South Charlotte communities is seeking a compassionate, experienced, and patient-focused Registered Nurse (RN) to join its clinical team. This is an excellent opportunity for an RN who enjoys working in an outpatient setting and building meaningful relationships with patients. The RN will work closely with providers and the care team to support preventive care, chronic disease management, patient education, clinical triage, and overall patient wellness . The ideal candidate is organized, clinically confident, and comfortable working in a fast-paced primary care environment while providing exceptional patient care and support. What You'll DoPatient Care & Clinical Support Provide direct nursing support to patients in a Primary Care setting. Perform comprehensive nursing assessments and obtain patient histories, symptoms, and medication information. Obtain and monitor vital signs and other relevant clinical information. Assist providers with examinations, treatments, and in-office procedures. Administer medications, injections, and immunizations within scope of practice. Perform or assist with EKGs, venipuncture, specimen collection, and point-of-care testing as needed. Review laboratory and diagnostic results and communicate pertinent findings to providers. Assist with medication reconciliation and medication management. Provide patient education related to medications, treatment plans, preventive care, and disease management. Respond to patient calls, portal messages, and clinical questions in a timely and professional manner. Triage & Care Coordination Perform telephone and in-office triage, assessing patient symptoms and determining appropriate next steps. Identify changes in patient condition and escalate concerns to the provider or appropriate clinical team member. Coordinate referrals, diagnostic testing, specialist appointments, and follow-up care. Follow up with patients regarding labs, diagnostic testing, treatment plans, and provider recommendations. Support patients with chronic conditions including diabetes, hypertension, hyperlipidemia, and other common primary care diagnoses. Assist with preventive care outreach and ensure patients remain up to date with recommended screenings and immunizations. Help coordinate continuity of care between patients, providers, specialists, and other healthcare professionals. Documentation & EHR Accurately document assessments, patient communications, interventions, and follow-up activities in the EHR. Review patient charts and identify outstanding labs, screenings, referrals, or other care needs. Maintain accurate and complete clinical documentation. Process medication refill requests and assist with prior authorizations as needed. Ensure provider orders and patient follow-up needs are completed appropriately. Clinic Operations Support efficient patient flow throughout the clinic. Assist with maintaining exam rooms, clinical supplies, and equipment. Follow HIPAA, OSHA, infection-control, and patient-safety standards. Participate in quality improvement initiatives and practice-wide clinical goals. Work collaboratively with Medical Assistants, providers, front-office staff, and other members of the care team.

Qualifications Required:

Active and unrestricted Registered Nurse (RN) license in North Carolina . Associate Degree in Nursing (ADN) required; BSN preferred. Current BLS certification through AHA or American Red Cross . Previous nursing experience in an outpatient, Primary Care, Family Medicine, Internal Medicine, or similar healthcare setting preferred. Strong clinical assessment, communication, organization, and critical-thinking skills. Ability to work independently while also functioning effectively as part of a collaborative clinical team. Strong computer and electronic health record (EHR) skills. Compassionate, professional, and patient-centered approach to care.

Preferred:

Previous Primary Care or Family Medicine experience. Experience with telephone triage and patient symptom management. Experience with chronic disease management and preventive care. Experience with medication management, prior authorizations, and referrals. Experience with quality measures and preventive health initiatives. Bilingual English/Spanish skills are a plus.
