Key Responsibilities:

Perform annual home health risk assessments for members of a Medicare Advantage health plan Review and evaluate patients' prior and current medical histories, chronic conditions, and medication profiles Educate patients on the management of their medical conditions and medications Identify care gaps and provide clear recommendations to bridge those gaps Complete all required documentation related to assessments and recommendations No prescribing of medications or development of treatment plans is required

Qualifications:
