Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage plan members Evaluate patients prior and current medical history, conditions, and medications Educate patients about their conditions, treatments, and medications Identify gaps in care and provide actionable recommendations to improve health outcomes Each assessment is focused on evaluation; you will not be required to create treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner license in NC Board certification through AANP or

ANCC MSN
