Job Description
Liberty Health seeks a compassionate Hospice RN to deliver high-quality, person-centered end-of-life care in home and facility settings. The Hospice RN completes comprehensive assessments, develops and updates individualized care plans, administers medications and treatments, and provides skilled symptom and pain management. This role coordinates closely with physicians, social workers, and other team members to support patients and families emotionally and clinically. The nurse documents accurately in the EMR, educates caregivers on disease processes and care techniques, and participates in on-call rotation and quality improvement initiatives. Responsibilities Provide skilled hospice nursing care in home and facility settings Perform comprehensive patient assessments and ongoing reassessments Develop, implement, and update individualized care plans Manage pain and symptoms using evidence-based interventions Administer medications and treatments safely and accurately Educate patients and families on disease process and caregiving Coordinate care with physicians and interdisciplinary team members Document all care and communication in the EMR in a timely manner Participate in on-call rotation, visits, and crisis intervention as needed Support quality improvement, compliance, and best-practice initiatives Required Skills Hospice nursing care Pain and symptom management Patient and family education Care planning and coordination IV therapy and medication administration Electronic medical record (EMR) documentation Palliative care principles Wound care Case management Interdisciplinary team collaboration
